Important reminders for filling out appointment forms:
- Your Social Security Number must be documented on the form in order to perform a credit check.
- The license of the state in which you will be selling group products must be included with the licensing material when mailed to the Home Office.
- If an assignment form is not included with the appointment material, commissions will be credited to the Social Security Number provided on the application. The assignment may be changed at anytime by submitting the assignment form. Please allow 4-6 weeks for processing.
