Important reminders for filling out paperwork:

  1. The license of the state in which you will be selling group products must be included with the licensing material when mailed to the Home Office.
  2. If an assignment form is not completed, commissions will be credited to the Social Security Number provided on the application. The assignment may be changed at anytime by submitting the assignment form. Please allow 4-6 weeks for processing.
